Adoption Application

Thank you for coming to adopt a pet! The decision to adopt a pet is an important one which we take very seriously. Filling out this form does not mean that you are required to adopt.

Please take a moment to read and carefully complete this application. If a field is not applicable to you, please complete it with "N/A".

In order to adopt a pet, you must:

• Be 21 years of age or older

• Be able to provide references and two forms of identification

• Consent to a home visit if it is deemed necessary 

• Agree to pay the donation (determined on an animal-by-animal basis; please ask before assuming)

In Our Hands Rescue reserves the right to deny any application. Once you have adopted an animal, the donation is non-refundable and will be considered a tax-deductible donation. Thank you for your patience and cooperation!
